Dynamic columns for a list/library.

I've a table in sql server with several columns in it, whenever i create a new folder in the list/library the folder needs to be created with value of the primary key column (a drop down with the values of the primary key columns) of the table and the other columns in the list/library must be taken from the rest of the columns of that particular row in the sql table. Folder names must be unique. Please tell me how can I do that. It's very urgent.

ex:
This is the sql table
OppID OppDate CustID PONo PODate
245dfd 23/2/09 cisco 454 04/10/09
45efre 20/5/09 seagate 344 04/07/09

When the user clicks on new folder in the list all the values in the oppId column must be displayed in the drop down to name the folder and the rest of the fields must be the column names for the list with the corresponding values in it.
